General Tips:

Pressure Sore Criteria: To be a candidate for reconstruction he must achieve compliance with the following.
  1. Demonstrate no tobacco use for 8 weeks or greater to permit healing of any surgical wounds
2. Demonstrate a hemoglobin A1C of less than 7.0 to permit healing of any surgical wounds
3. Demonstrate acceptable offloading out of hospital to include mattress support and seated support to permit healing of any surgical wounds
4. Have indicated operative fecal diversion to permit healing of any surgical wounds
5. Demonstrate a serum albumin of greater then 3.2 to permit healing of any surgical wounds

Peri-areolar Top Surgery

Uses superior based (omega) dermal-glandular pedicle.
Draws marking by measuring the rise (over the breast) and the run (across the chest). You can remove up to 3 cm of skin in each direction from the nipple. Sometimes the markings are oval and that's fine.
Purse strings at the end with a 0 prolene. Take very small bites and don't travel very far for a nice pursing effect. He pulls this stitch at 6 weeks.
