DIEP Pathway: POD0: NPO, ASA supp + ASA 81 daily, SQH, Q1H flap checks POD1: CLD no caffeine, Q1H flap checks, OOB to chair, d/c Foley, AM labs (d/c labs if stable) POD2: Regular diet + BCIVF, Q2H flap checks, OOB ambulation, encourage oxycodone POD3: D/c dPCA, Q4H flap checks POD4: D/c home with pain meds, Valium, ASA and 2 weeks ppx lovenox No caffeine for 1 month. All attendings want prophylactic lovenox on d/c x2 weeks
Lower ext path: Prior to OR: transition to heparin at time of consult, eval for CTA vs angio Day of OR: Hct 25-27 POD2: Start dangle protocol (unless in frame) POD3: Out of ICU, Q2hr checks
